Hello! My name is LaShaunte. I’m a Social Services Director at an intermediate nursing facility. Part of my job in assisting with discharge planning, providing resources for discharge. Unfortunately those resources are not all free and those that are or that are discounted have extensive waitlist and long waiting periods. Most of the clients I serve are coming from being homeless and have suffered from mental illness, a disability, abuses, addiction and trauma.

I am asking for assistance for the transitioning process from living in a nursing facility to living independently after completed treatments. Money would go to financing initial deposits for affordable housing and fees for initial furnishings through a local nonprofit. The client would be responsible for continued regular rent and utility payments.

URGENTLY: I have one client who has been called to move in next week but needs financial assistance with the deposit and furnishing fees.

I would like to keep this fund going for others as well but Urgently I want to make sure this current transitioning client can move in and not lose her spot on what has been a year and a half wait. If she is not ready when they call her she will be placed at the end of the list again.
