Support Vans recovery and final days at home

My sister and myself are raising funds for my father, Van Tyus, who has been battling heart failure for about 5 years now. A recent heart attack following failed dental work has left him in the hospital for the better part of a month. He is now at the point where he can no longer work or care for himself the way he would like and would like to spend his remaining time at home instead of in the hospital. We are looking to raise funds to help get him in-home care as well as cover medical expenses and basic needs. As he lives alone, he cannot be discharged until he has someone in-home to assist him throughout the day. As he is retired, he barely has enough income to cover his rent, and since he can no longer work, he has no means to generate any additional income to cover basic necessities, let alone the care he will need in-home. My sister and I do not live close, nor do we have the means to fund this on our own or stay with him full-time. We appreciate any help anyone is willing to give to assist with him being able to live out his final days in the comfort of his own home.
