My name is Racquel Bernard. I am raising funds for my mom, Pier Forbes. She has been diagnosed with Dementia, likely Alzheimer's. Her primary care doctor has advised that she needs 24-hour care. The funds raised with this GoFundMe will go towards her living expenses. She is currently in the hospital being transferred to a skilled nursing rehabilitation facility for short-term rehab. She is going to have to relocate from Port St. Lucie County to Polk County soon to be closer to my brother, Dominik. When she is discharged from rehab, she has to go into an assisted living facility. The ones we are researching costs an average of $2,500 a month. She will likely need memory care which averages closer to $4,000 a month.
