My name is Carlos, and I’m asking for help during a very sudden and overwhelming moment for our family.

My mom was recently discharged from her skilled nursing facility not because she recovered — but because Medicare stopped paying for her stay. She still cannot stand, walk, or transfer on her own. She requires a Hoyer lift, a wheelchair full-time, and assistance with every part of her daily care.

Even though she still needs skilled support, she had to be sent home. We were not prepared for this financially, physically, or medically.

Now that she’s home, we urgently need to create a safe environment so she can live with dignity and avoid further injury or hospitalization. This includes:

• Installing a safe access ramp so she can get in and out of the house

• Making home modifications for her wheelchair and lift

• Securing reliable medical transportation to and from dialysis three times a week

• Getting the equipment she needs for safe mobility and daily living

• Covering care-related expenses we did not expect

I’m doing everything I can to manage her appointments, coordinate her care, and handle the insurance issues — but the reality is that these costs hit all at once, and I simply cannot cover them alone.
