Hi, my name is Mark and I'm the son of Danuta, who passed away in a Long Term Care facility with complications. The Ministry of Long Term Care (Ontario) investigated the home where she resided and concluded that this home did not follow her care plan protocol. Unfortunately, this contributed to her suffering, ultimately leading to her passing.

We need a further investigation and a report done by professionals. This can be quite costly. We are asking for your help so we can continue this investigation process with the Long Term Care facility in hopes to find further explanations on why this happened. By doing so, we hope that we can finally conclude our suspicions so this will never happen again to others.
