Support Loyola Street Medicine-Housing Forward

The Loyola Street Medicine program strives to serve both the medical and non-medical needs of individuals experiencing homelessness. This population disproportionately suffers from acute and chronic illnesses, mental illness, and trauma, and lacks adequate access to primary care providers. Additionally, the homeless community has been particularly affected by the COVID-19 pandemic and the corresponding shelter-in-place orders.

We are partnering with Housing Forward, the only social service agency in west Cook County with a comprehensive "housing + services" approach for transitioning individuals/ families out of homelessness. They have been working since 1992 to provide long term solutions, impacting as many as 2000 lives every year. 

Our current work:
- delivering weekly breakfast meals
- assembling hotel to home kits
- conducting virtual health education programming for Housing Forward’s clients
- providing healthcare to clients
- assisting with daily COVID screening calls to help monitor clients’ health. 

We need your help to continue this much-needed effort, so please donate! A $50 donation can provide breakfast for as many as 30 people. 

All donations go directly towards meal deliveries, home goods for clients transitioning into permanent housing, hygiene products, and medical supplies.
