We believe it provides excellent information and much needed awareness about liver disease, liver failure and liver transplantation. My name is Trent Ford. I received the grace of a liver transplant on May 30th, 2020. The love of my life, Natalie Cohen, is my Caregiver and fought to save my life.

Over the last two years, we have been drawn to help others in a similar position- To share our experience and advice, to offer compassion and to help guide people through the process. It is an involved and taxing road and we have found success being there for others. At our core, we host support groups and provide ongoing one-on-one counseling, amongst other things.

The demand continues to grow and we are at a point where it is clear that there is so much more we could do with the right resources and the ability to devote our time more fully to this cause.

One major aspect of this, is the explosion of need for liver transplant due to alcoholism and other forms of addiction. This is only possible through careful guidance and a profound change in a person's life. This is a mandatory requirement from all transplant centers. Here we have found ourselves to be particularly effective.

Regardless of reason, a transplant life requires an embrace of great change- for both patient and caregiver. It is a marvelous fresh opportunity when taken as such. A second chance.

We are asking for help to finance the startup costs of forming our nonprofit- filing, legal, insurance, accounting and other related expenses. It will be named RELIVE with the Phoenix as our logo- renewal, rebirth, regeneration. A new life out of the ashes of our previous one- brighter, full of hope.

We would be honored if you could help us put this sincere Calling of ours into motion. We are delighted to say we will dedicate a page on our upcoming website to our 'Founding Donors'- anyone who can donate at this stage. Once granted 501(c)(3) status, charitable donations received will be tax-deductible retroactive to the date of our nonprofit’s formation.
