I never imagined I would be in a position where I’d need to ask for help, but life has brought my maternal uncle to this moment.

It’s been 5 years, in 2020 he had an accident.

On 6/7/2020, patient present with alleged Industrial injury. He was fallen into a cement hole (6 feet height) due to a trolley hit him from behind while cleaning. Post trauma, he was unable to ambulate, no LOC, unsure ENT bleed.

Upon arrival at ED, GCS 15/15.

Fast scan shows: free fluid at Marrison pouch, sliding sign present

Patient was sent for emergency OT for laparotomy and proceed for indication as intraabdominal injury:

Diagnosis:

1 liver injury + proximal transverse colon injury with mesentery injury

Underwent:

• Exploratory laparotomy + liver packing + proximal transverse colectomy on 6/7/2020

IOF:

= Liver segment VI / VII laceration, packing done with 2 abdominal pack (large)

= Several tear over proximal transverse colon with devascularized segment Proceed with transverse colectomy and with staplers and both ends drop into

abdomen

Rest of liver, GB, stomach, spleen, small bowel, large bowel normal Skin and subcutaneous layer of anterior abdomen was sheared off from musculature

• Relaparotomy + removal of liver packing + right hemicolectomy + cholecystectomy ileocolic anastomosis on 9/7/2020

IOF:

2 large abdominal packing removed

- Serious liver laceration at segment VI and VII → examine: no active bleeding, surgical x2 applied

Noted distal colonic stump healthy Proximal colonic stump unhealthy

Right hemicolectomy with STS anastomosis

Gallbladder distended, area over porch-stained with bile ? biliary / duodenal perforation

→ Duodenum kocherized and checked (no injury)

→ Cholecystectomy done, no CBD injury

Stomach, spleen small bowel, and rest of large bowel normal

After having lot of operations now my condition is even worse. Five years of medical struggle and losing money on operations is emotionally and financially exhausting. Now I only have 3 more months as the hospital and doctors all said I will not survive for long now. I have a family, my wife and my 13 years old son to look after.
