In need of help getting my dog to surgery

Hi, my name is Chloe and I am a German Shepherd mix with Chronic Ear Infections. I have been to the local vets numerous times over the years to get surgeries to help me but it's coming to a point where they are referring me to get surgery done in PEI or Halifax as the infection growth is way down in the canal where they cannot reach it. The first picture is when the growth is visible, but they can only do so much for me. There is noone on the island skilled enough for my surgery.

It was explained to me that I need to have my face opened up in order to get to the growth. In doing so I risk losing all feeling in that side of my face and become deaf, but it would SO worth. As I go on, I have also needed surgeries for my Hemotomas(?) (last picture) because where I flap my head around so hard, it causes the blood to pool in my ears and it needs to be drained. These pictures above happened within 8-10 hours while everyone was asleep.

I high anxiety and I do not like to travel in vehicles, I will get sick and therefore will require some medication to calm me down before I leave.

I will need to stay at a hotel for a few days because apparently they need to run some tests, and then do the surgery. I live in Cape Breton and Halifax is approx 5 hours away from me if we decide to go there.

I do not wish to be in pain any longer. We have tried everything from pills, to sprays, we have tried switching foods for allergies and nothing is helping me. When I get my local surgeries they last for about 6-8 months and then I'm back to scratching and flapping my ears. I just want to have the last years of my life relaxing. I am 8 years old and I need help. My parents are students and work part time jobs to help support just surviving and I have two big brothers ages 6 and 5 who love me very much but hate seeing me in my condition. I also have a fur brother who would be lost without me, he is my partner in crime and we cuddle alot, I am the one who gets the treats on the counter for him because he is too short,.

Please help.
