Help after medical issues

I am asking for help after a week of medical issues left me unable to return to my job the week after Thanksgiving, 2017. I did not have PTO available, so my week off went completely unpaid, and my bills are coming due in the next couple of weeks. The medical issues in question have also left me at a point where I must make fundamental change to myself and my life, and these funds will be contributing to the success of those goals as well. Anything you can spare would be greatly appreciated, because as of right now I'm unsure how I will be paying my dues.
