Help Our Beloved Mom Be Cared For In Her Home

I am writing on behalf of my mom. She had a stroke at the end of February, 2020. She has been in a Skilled Nursing/Rehab Facility since then. She REALLY wants to come home! She has made tremendous progress and is now able to do many things on her own.
While raising three kids she always worked full time, starting out as a chemist and then later working other jobs. My mom has always been there for others. Growing up, I remember that she would always cook a lot food for dinner, because we always knew that someone might drop by at dinnertime. Through out her life, my mom has always found the time and energy to help others. She always has the time for a friend in need, whether they needed her to take them to the market or to be a good listener. She has supported various causes; including the United Farmworkers Union, Trying to get Universal Child Care, LACER (an after school program), Echo Park Neighborhood Council, CARA (Medicare for ALL), Griffith Park Adult Center and working the elections.
Generally my mom is doing well. She would do even better and be so happy if she could return to her long time home. She has also been able to stay COVID free! She will need twenty four hour live in care/supervision. During this time in the Pandemic everything has been much more complicated. Some of what she will need is a live in caregiver, masks for all who enter her house, gloves, hand sanitizer and other PPE. We may also need to do some minor modifications like put in grab bars. I would like to bring her home in the next week or two as soon as we can have everything in place in order for her to stay safe. It's time for her to be in her home of almost seventy years!
Our whole family would be so appreciative if you could help out. Thank you so much for taking the time to read my mom's story.
