Help me get my medical card

I was diagnosed with Multiple Scleorsis in early 2022 and due to complications with the disease I lost the job I had at the time and I can't go back to work. I have difficulty doing a number of basic tasks because I'm stuck in a cognitive fog most of the time and my anxiety doesn't help. Getting this would be a the first step to recovering a lot for me.
