Ciao a tutti, devo fare un appello per me faticoso ma necessario... poco più di un anno fa la mia vita era perfetta, avevo comprato casa, avuto un bambino e vinto un concorso. Poi da un linfonodo ingrossato mi è stata trovata una metastasi da Melanoma. Vi lascio immaginare lo shock di una diagnosi del genere, a 33 anni, per me e per la mia famiglia. Mai però, siamo stati tanto uniti per combattere questa malattia.

Ora ho bisogno anche di voi e di chi non mi conosce per affrontare una terapia privata allo Sheba Medical Center in Israele in quanto al momento non ci sono altre opzioni terapeutiche nel nostro paese. Spero nella ricerca e vi ringrazio di cuore se vorrete darmi una mano, condividendo e partecipando a questa raccolta fondi.

La mia storia clinica inizia a dicembre 2021 con la rimozione di un linfonodo che è risultato sede di ripetizione neoplastica da melanoma. A febbraio 2022 si avviava il protocollo EBIN presso l’IFO Regina Elena di Roma con una terapia basata sulla combinazione di quattro farmaci che è proseguita per alcuni mesi.

A dicembre 2022 è stata riscontrata la progressione della malattia e così si è avviato un nuovo protocollo di cura presso l’IRCSS Pascale di Napoli. A marzo del 2023 la malattia è ulteriormente progredita e si è iniziata una nuova cura con altri farmaci che ha mostrato purtroppo solo una risposta parziale. A questo punto si è ravvisata quindi la necessità di un ulteriore cambio di terapia, ma al momento non si dispone di vere alternative valide.

Dopo molte consultazioni con i più importanti istituti europei che si occupano del melanoma, la soluzione migliore che è stata individuata è stata quella di ricorrere al trattamento TIL dello Sheba Medical Center in Israele, un trattamento immunoterapico di nuova generazione che si è dimostrato negli studi clinici una nuova promettente opzione terapeutica per le persone con melanoma metastatico.

Si tratta di una strategia di cura che prevede innanzitutto il prelievo del tessuto tumorale del malato. All'interno di esso oltre alle cellule tumorali sono presenti dei linfociti, quindi cellule del sistema immunitario, che tentano di attaccare e “uccidere” la malattia. Con la terapia TIL si estraggono dal tessuto tumorale del paziente queste cellule di linfociti già predisposte a riconoscere e uccidere le cellule tumorali, per moltiplicarle in laboratorio e successivamente infonderle nel paziente: l’obiettivo è quello di fornire al paziente tante “forze fresche" in grado di riconoscere ed attaccare la malattia.

Abbiamo avviato da circa un mese l’iter di valutazione inviando tutto il materiale allo Sheba Medical Center che ha preso in carico il mio caso e ha confermato la possibilità per il suo caso di essere sottoposta al trattamento TIL.

Vi chiedo aiuto per avviare il percorso terapeutico che purtroppo ha un costo molto elevato, oltre le nostre possibilità, e nello specifico prevede:

⁃ intervento chirurgico per prelevare il tessuto tumorale da cui estrarre l’infiltrato linfocitario

⁃ fase di rivalutazione pre-trattamento

⁃ reinfusione dei linfociti in combinazione con nivolumab, immunoterapia utilizzata in diversi tumori, incluso il melanoma.

Lo Sheba Medical Center è il più grande ospedale in Israele, tra i migliori e più avanzati ospedali al mondo e vorremmo con tutte le nostre forze poter avere questa opportunità.

Hello everyone, I need to make an appeal to you all, that is hard but necessary for me... just over a year ago my life was perfect, I had bought a house, had a baby and won a competition. Then from a swollen lymph node I was found a metastatic melanoma. I let you imagine the shock of such a diagnosis, at the age of 33, for me and my family. But never in our lives have we been so united to fight this disease.

Now I also need you and those who don't know me to undergo private therapy at the Sheba Medical Center in Israel as there are currently no other treatment options in my country. I place hope in medical research and I truly thank you if you can help me by sharing and participating in this fundraiser.

My clinical history begins in December 2021 with the removal of a lymph node that showed a neoplastic repetition from melanoma. In February 2022, I started the EBIN protocol at the IFO Regina Elena in Rome with a therapy based on the combination of four drugs which continued for a few months.

In December 2022 a progression of the disease was found and thus a new treatment protocol was started at the IRCSS Pascale in Naples. In March 2023 the disease progressed further and I began a new treatment with other drugs which unfortunately showed only a partial response. At this point, therefore, the need for a further change of therapy has arised, but at the moment there are no real valid alternative options in Italy. After many consultations with the most important European institutes dealing with melanoma, the best solution that has been identified was to resort to the TIL treatment of the Sheba Medical Center in Israel, a new generation immunotherapy treatment that in clinical studies has proven itself a promising new option for people with metastatic melanoma.

It is a treatment strategy that involves the removal of the patient's tumor tissue. Inside it, in addition to the tumor cells, there are lymphocytes, therefore cells of the immune system, which try to attack and "kill" the disease. With TIL therapy, these lymphocyte cells already predisposed to recognize and kill tumor cells are extracted from the patient's tumor tissue, multiplied in laboratory and then infused into the patient: the goal is to provide the patient with many "fresh forces" in able to recognize and attack the disease.

We started the evaluation process about a month ago by sending all the documentation to the Sheba Medical Center which confirmed the possibility for my case to undergo TIL treatment.

I ask you for help to start the therapeutic process which unfortunately has a very high cost, beyond our possibilities, and specifically provides for:

⁃ surgery to remove the tumor tissue from which to extract the lymphocytic cells

⁃ pre-treatment re-evaluation phase

⁃ lymphocyte reinfusion in combination with nivolumab, an immunotherapy used in various cancers, including melanoma.

Sheba Medical Center is the largest hospital in Israel, among the best and most advanced hospitals in the world, and I’m trying to do everything possible to have this opportunity.
