The NPT Acute Clinical Team (ACT) provides exceptional short-term care for frail patients in the Neath Port Talbot area, allowing them to remain in the comfort of their own homes instead of facing hospital admissions. Led by dedicated Nurse Practitioners and supported by Consultant Geriatricians, this team delivers a dynamic approach to urgent medical responses in our community, ensuring that patients receive the care they need where they feel most secure.
