Help with my independence, service dog fund

Battling mental illness is a daily roller coaster of emotions and even physical discomfort.  I have always fought silently and independently. I had a job, a church community, and friends in my life.  But in the last couple years my Anxiety has gotten more severe, and my depressive disorder has pushed me down hard. I started calling into work often instead of striving for the dependability I was known for. Then in late September of 2018, I was on the brink of suicide. Luckily, even though I lived alone, a very close friend took me to our crisis center. After a few days of support, learning techniques to handle the emotions I was released. Scared to live alone I moved in with my boyfriend. I got on med management and started to feel safe again. But then work started to take a toll on me again, I tried and tried push through by changing medications and even going to therapy to no avail. May 2019 I quit my job due to ever increasing inability to get consistent sleep, to eat regularly, to go out due to increasing severity of my anxiety, and bad muscle fatigue. I am applying for disability so that I can hopefully find a balance and one day return to work. That process has pushed my anxiety through the roof. Now I am struggling to get out of bed, do daily hygiene tasks, help with housework, and even take my meds consistently (due to weird sleep schedules). That gets us to now. My med Dr and I both feel that a service dog would be the right amount of assistance and drive to get me on track while we work on med management.  I found an agency where I am currently applying. They actually have a Cavalier King Spaniel that I'm hoping would be the best fit. Costs will cover, the dog, any supplies and gear needed, hotel costs to go work with her, and food/grooming costs (until disability gets approved).
