For ten years, The Care Plan has walked beside individuals and families through life’s most challenging transitions from illness and aging to caregiving and grief. Founded to center compassion, dignity, and equity, we’ve helped countless people feel supported, informed, and empowered as they navigate the complexities of health, family, and the future.

Today, the need for care has never been greater. Rising fear, shrinking resources, and deepening inequities mean that too many in our community are facing these challenges alone. That’s why we’re launching The Care Plan Community Care Fund to ensure that those who cannot afford care navigation still receive expert, compassionate support through health crises, caregiving responsibilities, and end-of-life planning.

Every contribution directly funds free and reduced-cost care for those most in need. And if you can’t give financially, you can still make an impact by sharing our campaign and spreading the word. Because care shouldn’t be a privilege, it’s a promise we make to one another.
