2021 Annual Fundraiser to Support Seniors

You can make a difference by donating today to support seniors in our community.

Together we can do much more, but we need your help.

Your donation will enable us to meet critical individual needs, to improve and expand our services, and develop new innovative approaches to care. Like offering weekly wellness calls to check on vulnerable seniors, especially those living alone.  Training seniors to use smart devices to reduce the digital divide and combat social isolation. Providing door-to-door  rides to seniors in rural parts of the county. Connecting them to appropriate resources as they encounter new phases in their life.

Services that will help elders—and people with disabilities—live their best lives.

Since 1973, we have provided services and support to seniors in Santa Clara County. At Sourcewise, we continue to support seniors and caregivers as we expand our reach to include persons with disabilities and all adults in Santa Clara County. We aim to inform, educate, and prepare all adults within Santa Clara County to better navigate their health and life options.

Our mission is to provide adults and their caregivers the tools and services they need to effectively navigate their health and life options.

At Sourcewise, we collaborate with Santa Clara County, state, and local networks to provide a streamlined approach to service and support systems. We empower individuals by providing access to information, allowing for personal choices, and continued independence. We strive to create a community-based system of care that crosses city boundaries, income levels, geography, and special interests.

We invite you to visit our website to learn more about our programs and services, and ways to stay involved.
