My name is Josey Trawick, former owner of Shoals Game Room in Muscle Shoals, Alabama. I have the following medical conditions. My disability attorney has requested a hearing with the Social Security Administration.
1. Leukemia (Chronic Myelogenous Leukemia) - Chemo Daily
I am treated at Clearview Cancer Center of Muscle Shoals.
2. Meniere's Disease -Vertigo, Poor balance, tinnitus
Formerly treated by Dr. Kenneth LeMaster and Keller ENT
3. Ocular Histoplasmosis - I have blindness in both eyes from mutated/bleeding sub retinal blood vessels. I am treated at The Retina Specialist of North Alabama in Florence by Dr. Scott Dorin. I have had 42 (Avastin) injections in my eyes to keep my vision. Avastin is a 1:500th dose of chemotherapy normally used in the treatment of cancer. This is a degenerative eye disease which has caused all central vision loss in my left eye and partial central vision loss in my right eye.
4. PTSD Post-Traumatic Stress Disorder
5. Major depressive disorder
6. Generalized Anxiety Disorder
7. Obsessive Compulsive Behavior
8. Insomnia
I am treated by Integrated Behavioral Health (IBH) of Muscle Shoals by Mallory Avant and Dr. Timothy Whalen for 4-8
I was literally abandoned after I closed Shoals Game Room.
My wife had the only income and left the marital home March 13th of 2025. We are going through a divorce.
I do not have health insurance. I was denied coverage on Healthcare.gov. I also have no income other than the support from my family, friends and community.
My mortgage is In jeopardy of foreclosure, I was approved for loss mitigation (forbearance) through December 1st 2025. I now have until Mar 01 2026 before the bank can forecloses on my home. I am responsible for paying utility bills, car insurance, gas, medical bills, and medicines.
I have applied for patient assistance at Clearview Cancer Center and I was approved for 100% with a $0 co-pay. I am self pay at the Retina Specialist of North Alabama to monitor and treat the retinal vessel mutation, bleeding and loss of vision. I am also self pay at my mental health provider, IBH of Muscle Shoals.
I have officially been deemed unable to work due to my health conditions by IBH. I am unemployed and have been denied Social Security Disability many times since in spite of everything I am going through. The last denial was a few months ago and yes I do have an attorney. I am proud to say that Congressman Mr. Robert Aderholt is now aware of my disability case.
I have caseworkers with the Alabama Institute for the Deaf and Blind in Tuscumbia (AIDB), Easter Seals in Muscle Shoals, and the North Alabama Rehabilitation Center in Muscle Shoals.
With all of the conditions I have, I can't be a liability to an employer or myself. I have two daughters, one of which is living with me part time. I have always worked and went above and beyond to provide for my family.
- I'm so thankful for the food that was donated to us from the Dream Center. And I am thankful for everyone who donates to them.
- I want to say thank you for the NACOLG specifically Paula Pardue for helping me find extremely low cost medications. For only $220.00 I am receiving a 90 day supply of my 5 medications including the chemo pill.
- I want to say thank you to Community Action for assisting with my utility bill.
- I want to thank Colbert County DHR for EBT/SNAP benefits.
- I also want to thank the Long-Lewis Foundation for the $250 donation credited to IBH to help pay for my treatment.
- I want to thank my previous employer WBTG Radio station in Sheffield for their generous love and support.
- I want to thank my community for helping me through this rough time, I am truly depending on you.
