My name is Andrew, I am an Uncle and I am representing for my family. On Sept 3rd, Jack Slemmer, our son, brother, father, nephew, and grandson, passed away at his home. Jack was only 36 years old. He was known in our family as the joker, the animal lover, a motorcycle enthusiast, and someone who adored his children and friends.
