I hope this letter finds you well. My name is Dustin Cook, and I am writing to you on behalf of my son, Forrest Cook, who has been diagnosed with muscular dystrophy. Forrest Cook is a 8-years old boy, he is bright and energetic child who loves baseball and the outdoors. Despite his condition, he remains positive and determined to live his life to the fullest. However, the challenges posed by muscular dystrophy are immense, and the costs associated with his care and treatment are substantial.
We are seeking financial assistance to help cover the costs of home modifications(ramps/stair assistance) along with proper transportation resources as needed. These resources are crucial in ensuring that Forrest can maintain his independence and continue to engage in activities that bring him joy and fulfillment.
We have explored various avenues for support, including insurance and government programs, but unfortunately, these sources do not fully cover the expenses required for Forrest's care. Therefore, we are reaching out to your organization, which has a commendable history of supporting families affected by muscular dystrophy, in the hope that you can assist us during this challenging time.
