Support for Ursula - OLAPARIB

Hey there,

My name is Stella and I am starting this fundraiser in support of my beautiful former composition tutor and dear friend, Ursula Caporali. I’m hoping to raise funds to help Ursula and her husband, Paolo, cover the cost of a potentially life saving treatment.

Ursula was diagnosed with Stage 3 lobular invasive breast cancer in May, 2019, and has since undergone a series of exhaustive treatments; mastectomies, chemo, radiation, a plethora of medications, and multiple operations - including brain surgery. The list goes on. To date, all conventional therapies and procedures have proven to be ineffective in controlling the rapid progression of the cancer, and her current diagnosis is Stage 4 metastatic (spread into the bones, brain and lymph nodes) breast cancer.

As a last resort, Ursula was recently prescribed the AstraZeneca drug Olaparib and has been taking it since June, 2021. In just 2 months, Ursula’s oncologist has observed improvements in her blood cancer markers and an overall positive response to the drug. For the first time in months, Ursula has been able to feel a little bit more like her spunky self, even finding the energy to gift her Facebook friends with the most Ursula-esque Olympic commentary possible - a combination of heckling (the Americans of course) and hearty cheers for the Italians and Aussies (that were no doubt heard in Tokyo!!).

Despite the use of Olaparib as a recognised treatment in the US, Europe, South America and Japan, however, it has not yet been approved in Australia as a treatment for Ursula’s specific, less common form of breast cancer, and to pay out of pocket is costing Ursula and her husband Paolo approximately $7,000 per month. Without financial aid or subsidies, for Ursula to just stay alive for one more year will cost around $84,000. 3 months’ worth of the drug has already cost them $21,000.

The type of cancer that Ursula has is caused by the mutation of the PALB2 gene, and Olaparib is currently the only form of treatment that has shown signs of success in actually slowing the cancer. It works by silencing the mutated PALB2 gene and thus controlling the metastatic sites of the body (the brain, bones, lymph-nodes etc). Ursula’s doctors believe that currently, her best and only chance of survival and potential recovery is to remain on the Olaparib for the foreseeable future. The drug is therefore absolutely crucial in affording Ursula invaluable time, and her team believes that such time may then allow them to find an immunotherapy with long term potential for survival. 

Ursula has already sought financial support from the Australian Government, from AstraZeneca, from her current hospital, and from countless organisations. She has fought incredibly hard to exhaust all options currently available but has been repeatedly denied on the grounds that the TGA has not yet approved the medication for her less common PALB2 gene mutation and she therefore cannot meet the narrow eligibility criteria to receive subsidies from the PBS. Furthermore, there are currently no running trials for her to possibly join. To sit and wait for the drug to be approved in Australia requires time that Ursula simply does not have.

To spend years exhausted and battling an aggressive form of cancer is already an unimaginably difficult reality; to then have to pay huge sums of money out of pocket, just to survive, is a burden that no one should have to bear, and one that this fundraiser and your generous contributions will help to alleviate. 

Ursula moved from Italy to Australia in 2009, completing her PHD in 2014, and has since worked as a casual staff member at the University of Sydney (Conservatorium University and Highschool of Music), as a composer, composition tutor, and piano teacher. Her passion for all things musical is enormous and incredibly infectious. It is evident in the face-splitting grins of her little students as they perch, legs swinging, on the piano stool, engrossed in a duet with her; in her very composer-appropriate, can’t-be-tamed hair; even in the sing song voice she used to (lovingly) threaten and bribe me and my peers with when we weren't behaving in class - “Stellaaaa, I kill you!!! Only tiramisu for those who work!!” Those who know her would agree that this fierce passion extends to all things art and people and just life - she is vivacious, warm, and exceedingly generous. She is the first to fight for you and the last to give up on you. Her will to not just live, but to live fully and fiercely and wholeheartedly, even after all she has been through, is nothing short of extraordinary, and this drug has the potential to not only extend her time but to allow her a better quality of life.

I am asking for your generous support in hope that your contribution may relieve some of the financial pressures on Ursula and her family so that she can focus on fighting this disease - and just feel a little bit less alone in this already crazy and financially challenging time of lockdowns and physical distance from her beloved family and friends around the world. 100% of the proceeds will go directly to Ursula and to covering the costs of treatment.

Ursula è una persona straordinariamente generosa e piena di vita che considero mia sorella e amica insostituibile. Impossibile non amarla e non carpire da lei il valore della vita. Una persona rara di inestimabile valore . 
Purtroppo le è stato diagnosticato un carcinoma mammario  di stadio 3 nel maggio 2019 e da allora ha subito una serie di trattamenti invasivi;  mastectomie, chemio, radiazioni, una pletora di farmaci e operazioni multiple, inclusa la chirurgia cerebrale.  L'elenco continua.  Ad oggi, tutte le terapie e le procedure convenzionali si sono dimostrate inefficaci nel controllare la rapida progressione del cancro e la sua attuale diagnosi è di cancro al seno metastatico di stadio 4 (diffuso nelle ossa, nel cervello e nei linfonodi). 

Come ultima risorsa, a Ursula è stato recentemente prescritto il farmaco AstraZeneca Olaparib e lo sta assumendo da giugno 2021. In soli 2 mesi, l'oncologo di Ursula ha osservato miglioramenti nei suoi marker di cancro del sangue e una risposta complessivamente positiva al farmaco. L'uso di Olaparib come trattamento é riconosciuto negli Stati Uniti, in Europa, in Sud America e in Giappone, tuttavia, non è stato ancora approvato in Australia come trattamento per la forma di cancro al seno specifica e meno comune di Ursula .Deve pagare di tasca propria questo medicinale che  sta costando ad Ursula e a suo marito Paolo circa 7.000 dollari al mese.  Senza aiuti finanziari o sussidi, per Ursula rimanere in vita solo per un altro anno costerà circa $ 84.000.  Il valore di 3 mesi del farmaco è già costato loro $ 21.000. 

Il tipo di cancro che ha Ursula è causato dalla mutazione del gene PALB2 e Olaparib è attualmente l'unica forma di trattamento che ha mostrato segni di successo nel rallentare  il cancro.  Funziona silenziando il gene PALB2 mutato e controllando così i siti metastatici del corpo (cervello, ossa, linfonodi, ecc.).  I medici di Ursula ritengono che attualmente, la sua migliore e unica possibilità di sopravvivenza e potenziale recupero sia quella di rimanere sull'Olaparib per il prossimo futuro.  Il farmaco è quindi assolutamente cruciale per concedere a Ursula tempo prezioso e il suo team ritiene che tale tempo possa consentire loro di trovare un'immunoterapia con un potenziale di sopravvivenza a lungo termine. 

Ursula ha già cercato sostegno finanziario dal governo australiano, da AstraZeneca, dal suo attuale ospedale e da innumerevoli organizzazioni.  Ha lottato incredibilmente duramente per esaurire tutte le opzioni attualmente disponibili, ma è stata ripetutamente negata sulla base del fatto che la TGA non ha ancora approvato il farmaco per la sua mutazione del gene PALB2 meno comune e quindi non può soddisfare i ristretti criteri di ammissibilità per ricevere sussidi dalla PBS  .  Inoltre, al momento non ci sono prove in corso per lei a cui potrebbe partecipare.  Sedersi e aspettare che il farmaco venga approvato in Australia richiede tempo che Ursula semplicemente non ha. 

Passare anni esausti e combattere una forma aggressiva di cancro è già una realtà inimmaginabilmente difficile;  dover poi pagare ingenti somme di denaro di tasca propria, pur di sopravvivere, è un fardello che nessuno dovrebbe sopportare, e che questa raccolta fondi e i  vostri generosi contributi contribuiranno ad alleviare. 
Ursula si è trasferita dall'Italia in Australia nel 2009, completando il suo dottorato di ricerca nel 2014, e da allora ha lavorato come membro dello staff a progetto presso l'Università di Sydney (Conservatorium University e Highschool of Music), come compositrice, tutor di composizione e insegnante di pianoforte.  La sua passione per tutto ciò che è musicale è enorme e incredibilmente contagiosa.  Chi la conosce sarebbe d'accordo sul fatto che questa grande passione si estende a tutto ciò che riguarda l'arte, le persone e la vita: è vivace, calorosa ed estremamente generosa.  È la prima a combattere per te e l'ultima a rinunciare a te.  La sua volontà non solo di vivere, ma di vivere pienamente, ferocemente e con tutto il cuore, anche dopo tutto quello che ha passato, è a dir poco straordinaria, e questa droga ha il potenziale non solo di prolungare il suo tempo, ma di consentirle una migliore qualità di  vita.
