My name is Felicia. I am the youngest sister of Florence Le, who tragically passed away under Liverpool Hospital’s Care Level 1 supervision.
Florence was admitted to the mental health ward of Liverpool Hospital on 17 February 2024. She was immediately transferred into Care Level 1 as she was discovered attempting suicide at the mental health ward on 20 February 2024. Our family was reassured by Liverpool Hospital that Care Level 1 meant 24-hour/7-day care.
Our family entrusted Florence’s safety into the care of Liverpool Hospital, and we were promised that she would have constant supervision. On 26 February 2024, Florence was unsupervised for 20 minutes and tragically took her own life. The hospital spent 30 minutes resuscitating her. When we arrived she was breathing with the aid of ventilators and was declared brain dead.
Now, as our grieving family attempts to cope with this devastating loss, we find ourselves facing not only the emotional toll of mourning Florence but also the imperative need to seek justice for the substandard care she received.
We are reaching out to you, our community, to rally support and justice for our family as we embark on legal proceedings against the hospital responsible for her inadequate supervision and tragic death.
Funds raised will be utilised to cover legal fees, ensuring that our family has the resources needed to pursue a fair and comprehensive investigation into the circumstances surrounding her lack of supervision, and ultimately her death.

Florence was vivacious, full of life, kind-hearted and extremely hard-working. Her absence will be profoundly felt by all who were touched by her kindness, compassion, and generosity. She had a remarkable ability to make others feel seen and heard, offering solace and support to those facing their battles. Florence was also a music teacher at Kingsgrove High School and a performer at The Star Sydney. She also leaves behind 20 private piano students. Florence is survived by her grieving parents, her two sisters and her beloved two-year-old son.
