My name is Adam Jacobs. I am a husband and father of four, born and raised in Michigan. For the last year, I have been experiencing immense pain as the result of multiple herniated discs in the cervical region (C3-C7), as well as stenosis of C5 and C6. After three hospital visits and a few follow-ups, we discovered the degeneration of my spine has increased. This caused the nerve root for my right arm to be compressed, causing slight numbness, pain, muscle spasms, and tremors. Severe pain is a constant. After X-rays, an MRI, and a CT scan, and meeting with the surgeon, I am set up tomorrow morning to go in for surgery.

We have been charged $250 for every visit to the hospital, and the surgery is $1,000. We blew out a tire and a strut on our car during all of this insanity and driving back and forth, which killed us financially. I was forced to take medical leave from work due to pinching the nerve in C6 while on shift. My wife can't work due to having an 8-month-old to feed, various therapists for all four of our kids, and trying to get me to my appointments. I married this woman and have done everything to provide for her and our children. I promised stability, and I am now running out of time to keep the bills current.

We have filed through the state for assistance and received none. Requested from various churches, nothing. Not only do I need to fund my surgery, I have to save our home from eviction and keep the lights on. If there is anything you can help with, please reach out to me at the number listed. Thank you.

**Update** surgery was this morning at 6 am. It was successful and I'm now heading home to rest. I'm in excruciating pain, but I will do my best to answer any questions you may have
