A SOAP note is the basic unit of medical documentation. "SOAP" stands for Subjective Objective Assessment Plan. The four elements provide structure during a clinical visit and a narrative record after the encounter.
The SOAPnote Project is an effort to improve collaboration on note templates, checklists, and clinical calculators.
For every donation of $6, we'll arrange 1 year of ad-free access to the site. We'll connect your donation to your registered user account at The SOAPnote Project.
How it works: After you make a donation (anonymous or otherwise), we link it to your account to set up ad-free access. You will receive an email once this has been completed – please allow up to 24 hours for this process to occur.
To help us match your account to your donation, please feel free to email us your user name to be sure we get it right.
You don't need to make a donation to use the site. All of the features of the site are free to use for everyone. We're adding new features regularly and users are adding new content every day.
