I am a RN diagnosed with Hodgkins Lymphoma so naturally I had to organize this in SBAR...
Situation: I have had a cough without explanation or resolve since January 2013. Following a chest biospy in mid-August, I was diagnosed with Hodgkins Lymphoma.
Background: Paternal grandfather had Non-Hodgkins Lymphoma 2 years ago. No significant past medical history.
Action: Fertility consultations to investigate egg freezing.
Have port placed followed by ABVD Chemotherapy once every other week for 6 months. Continuing working as long as my health isn't jeopardized.
Recommendation: Let's Beat This! I want to make cancer survivor status.
