duchenne muscular dystrophy

Hi my name is Paras Patel , I was diagnosed with duchenne muscular dystrophy when I was 13 years old. Over the years I have lost ability to walk and now my arms are getting damaged and weak. I can NOT use manual wheelchair due to no arm strength . I have been able to get a power wheelchair but unable to leave my home since I do not have a vehicle to transport my power chair. Have to purchase a vehicle that has a ramp or install one in my van so I can travel to my doctor appointments . My heart has become dilated and I had to get a heart transplant 5 years ago. I am unable to transport myself without a power chair and vehicle with ramp. I am on disability can’t cannot work due to weakness of the limbs. Any amount would be much appreciated, so I can get a ramp installed and go to my transplant doctors appointments.
