Young Disabled Mother Needs Help

Hello, my name is Kim Holt. I have a niece, her name is Nichole. She is 28 years of age and the mother of a vibrant 5 year old little girl.
Nichole is living in a nursing home because she is disabled and unable to care for herself or her daughter. Currently, another family member is caring for her daughter but they are far away from Nichole and it does not allow to see her precious daughter as she would like.
My sister and I are trying to help her to get to Washington where she will reside in a nursing home that will provide the 24 hour care that she needs and her daughter, our grand niece, will live with my sister.
This is a major undertaking and as we have begun to assess the costs we realize it is beyond our means. We want Nichole to be close by and as interactive as she can be with her daughter!
Nichole has health care (Medicaid and Disability) in Georgia but as you may know, this does not travel with her. She must re-apply in Washington when she gets there and it takes 30-60+ days to get it. We will also need medical transport to the airport in Georgia and medical transport from the airport in Washington to her new residence.
I am only asking for what we need to cover her living expenses for 3 months and the medical transport
