I’ve never done one of these before, and I don’t know what’s required, so let me share the whole story…

After I injured my back, my old primary care doctor refused to fill out the paper work necessary for me to keep my job, and I was also denied temporary disability.

I’ve been doing physical therapy at home, because I can’t afford anything else. I went once, and have been trying to keep up the exercises they gave me. Sometimes those make my back worse instead of better so I’ve cut back to lighter routines. For three months now I’ve needed to use a cane to walk, and I’ve been recently fired from my job.

In those three months it’s taken me to find a new doctor and take the next steps towards recovery ( scans, x-rays, pain management injections, and most recently a cardiologist recommendation for black out spells... ) I’ve depleted the safety net of funds.

Now I’m stuck, and have no way to pay for rent, let alone money required to get myself well enough to find a job again. I’m going to try for unemployment to relieve some of the financial burden, but my biggest worry is that even with that it won’t be enough for rent, my car payments, bills, and treatments…

I can’t fall back on any family, because I only have my mom, and she’s already on disability and is going through this situation with me since we’ve always lived together. I used to be the person who could tough it out at work, and now I don’t know what to do.
