Pay off Chase home equity

Our home loan suddenly came to term and a final payment is due on the 22nd. We have paid about $300 monthly for 20 years. Chase bought out the original bank and didn't report the loan or the payments. We've tried for a loan for this final payment but were denied because our score is too low without the payment history our home loan lender should have provided. Now full payment is due Thursday. We didn't have even a 2 week notice the 20 year term was up. Hardest thing ever to ask for help. Please, if you can, help.
