Med Treatment for Pirate,the turtle

Pirate, the yellow-bellied slider needs urgent medical treatment. I found him this morning, in a very rough shape. He has numerous bites on his hind leg, serious infection, and is covered with maggots. He needs emergency wound cleaning, antibiotics and other treatment, see bill attached. I need help to treat him.
