Dental surgery needed now

In brief, I had some truly awful dental work done some years ago which has left my lower jaw/teeth with virtually no bone or gums.  As such, they are either falling out or need to be extracted! I have worked out a payment plan with my periodontist, which will leave me with having to find a minimum of $1,000. per month, for roughly 2 years to have the needed work completed. My perio is a good guy, but they will not even start the reconstruction until I've made payments. I have an entry level job which covers my modest usual expenses, but I'm not in a position to take out any kind of loans.  I don't own a home, I have no car and no "collateral" to offer.   My insurance and small savings are going towards the initial extractions, but I won't be able to continue unless I can assure them of ongoing payments.
