Moving and medical needs

After 49 years of searching for my biological family I finally found them. So I decided to move back to Michigan to be closer to them. Shortly after my car had broken down and only a few months later the covid virus came. With having health issues, heart disease and diabetes, I needed to quarantine myself. I recently found work but it's only a couple days a week and only a few hours a shift being paid cash. I'm behind in rent and being evicted with no where to go and no money. So I'm asking for help so I can move back to Missouri and get into a doctor so I can get back on my meds that I need. I haven't been able to afford them either. If you're willing to help I would more than appreciate it.
