The Ohio State Highway Patrol responded to a crash around 5 p.m. Monday on Fay Road.
Authorities said Vivian Miracle, 24, of Goshen, was driving westbound when she drove off the right side of the road and struck a tree.
Miracle and her rear-seat passenger, Aliyah Miracle, 4, needed mechanical assistance to be freed from the vehicle.
Both were transported to Bethesda North Hospital.
Aliyah Miracle died at the hospital. Vivian Miracle has incapacitating injuries, authorities said.
Police said both were wearing safety restraints.
