My name is Abraham David Guix, I am 30 years old, and I am from Costa Rica, where I currently live. I am a very adventurous person who likes to live one day at a time because I have a great zest for life.

In 2018, I was diagnosed with deep vein thrombosis in my left leg. I had been playing soccer and my left leg cramped up. Days passed and my leg remained swollen and cramped, so I decided to get a venous ultrasound, also known as a Doppler ultrasound, which my mother's friends recommended. They observed a clot in my left leg. I immediately had to go to the hospital and underwent a series of tests. I was prescribed Warfarin, an anticoagulant to dissolve the clots, and so the years passed.

THE BEGINNING OF MY NIGHTMARE

In 2022 I returned to the hospital for admission, this time due to unbearable chest pain. I would describe it as if something inside was puncturing me. I couldn't move, I had extreme fatigue where I couldn't walk even 100 meters because I was suffocating, it was so painful I couldn't breathe. I was admitted twice that year for the same reason. They performed several tests during that time where they detected that what I had was Thromboembolic Pulmonary Hypertension (group 4).

In 2023, I was hospitalized again. After that stay, I was given numerous follow-up appointments with top specialists at San Juan de Dios Hospital: pulmonologists, cardiologists, rheumatologists, and a peripheral vascular surgeon. I was also prescribed daily cardiac rehabilitation because pulmonary hypertension is a rare disease caused by a pulmonary embolism, and although physical exertion is difficult, it must be done to strengthen the respiratory system.

In 2024 I was hospitalized again because the clots did not dissolve and turned into scar tissue, which caused a blockage in the pulmonary artery that connects the heart to the lungs. This caused the right side of the heart to have to work harder to pump more blood, and in turn it grew and caused damage.

My health was steadily worsening. My pulmonologist told me I had to use home oxygen 24/7 and, in addition, a triple therapy (riociguat, macitentan, and treprostinil). These medications are very expensive; two are pills, and the other is a machine connected subcutaneously to the skin. The side effects are very strong, ranging from diarrhea, vomiting, headaches, joint pain, heartburn to jaw pain. Every week they had to increase my dosage for it to be more effective. To get this triple therapy, I had to go to the highest authority in my country so the judges could approve it, and I had to wait almost seven more months for approval since the health system in my country didn't grant it easily. Those seven months felt like years to me, and my health deteriorated even further.

As time went on, the triple therapy was helping me, however, my pulmonologist told me that there was a cure for my disease, and that cure was a highly complex surgery called "pulmonary thromboendarterectomy", a procedure that is only performed in a few parts of the world and unfortunately not in Costa Rica, and that's where the journey with my partner and my family to investigate began.

We started getting quotes and finally it was possible in Argentina. The surgery costs $75,000, plus adding the travel expenses for airfare, lodging, food, transportation, and post-operative care, it comes to approximately $85,000. I know it won't be easy, but with the help of each of you, with whatever you'd like to donate, I would be very grateful. Every colón or dollar helps the cause. I really want to live.

In Costa Rica there is a Pulmonary Hypertension Association that has helped me a lot, from psychological support to financial assistance. Sometimes I give motivational talks so that my testimony can help other patients who suffer from this rare disease.

Mi nombre es Abraham David Guix, tengo 30 años de edad, soy de Costa Rica donde actualmente vivo. Soy una persona muy aventurera y que le gusta vivir un día a la vez ya que tengo muchas ganas de vivir.

En el 2018 me detectaron una trombosis venosa profunda en mi pierna izquierda, ya que estaba jugando fútbol y la pierna izquierda se me arratonó, pasaron los días y mi pierna seguía hinchada y arratonada, por lo que decidí ir hacerme una ecografía venosa conocido como examen de doppler la cual amigos de mi mama le recomendaron, me observaron un coágulo en la pierna izquierda, de inmediato tuve que ir al hospital y me realizaron una serie de exámenes, me medicaron Warfarina, un anticoagulante para deshacer los coágulos, y así pasaron los años.

EL COMIENZO DE MI PESADILLA

En el 2022 volví al hospital para ser internado, esta vez por un dolor en el pecho insoportable lo describo como si estuvieran perforando algo en mi interior, no podía moverme, tenia una fatiga extrema donde no podía caminar ni 100 metros porque me ahogaba, no podía respirar ya que era doloroso, me internaron 2 veces ese año por el mismo motivo, me realizaron varios exámenes en ese periodo, donde detectaron que lo que tenia era HIPERTENCION PULMONAR TROMBOEMBOLICA del grupo 4.

Nuevamente en el 2023 volví a estar internado en el hospital, luego de ese internamiento me asignaron muchas citas de seguimiento con los mejores especialistas del Hospital San Juan de Dios: Neumólogos, Cardiólogos, Reumatólogos y Vascular Periférico. Tambien me mandaron a rehabilitación cardiaca todos los días ya que la hipertensión pulmonar es una enfermedad rara causada por una embolia pulmonar y aunque cueste hacer esfuerzo fisico hay que realizarlo para fortalecer el sistema respiratorio.

En el 2024 vuelven a internarme debido a que los coágulos no se disolvieron y pasaron a ser tejido cicatrizado, lo que genera un bloqueo en la arteria pulmonar la cual conecta el corazón con los pulmones, esto provoca que el lado derecho del corazón deba trabajar con sobre esfuerzo para bombear más sangre y a su vez crece y genera un daño.

Cada vez empeoraba mi salud, mi neumólogo me indicó que tenia que usar oxigeno domiciliar las 24/7 y adicional a eso una triple terapia, (riociguat, macitentan y teprostinil) unos medicamentos que son muy costosos, dos de esos medicamentos son pastillas el otro es una máquina que va conectada subcutánea a la piel, donde sus efectos secundarios son muy fuertes, desde diarrea, vómito, dolor de cabeza, de articulaciones, acidez hasta dolor de mandíbula, cada semana me tenían que subir la dosis para que me hiciera mejor efecto, para lograr conseguir esta triple terapia, tuve que acudir al ente de máxima autoridad en mi país para que los magistrados pudieran aprobar mi triple terapia, y esperar casi 7 meses más para la aprobación ya que el sistema salud de mi país no los daba tan fácilmente, en donde esos 7 meses para mi eran años y mi salud se deterioraba aún más.

Conforme pasaba el tiempo, la triple terapia me iba ayudando, sin embargo, mi neumólogo me dijo que para mi enfermedad había una cura, y esa cura era una cirugía altamente compleja llamada "tromboendarterectomía pulmonar", procedimiento que solo se realiza en pocas partes del mundo y desafortunadamente en Costa Rica no la hacen, y ahí fue donde empezó la travesía con mi pareja y mi familia a investigar.

Empezamos a cotizar y finalmente en Argentina se podía lograr. La cirugía tiene un costo de $75.000 y sumando los viáticos para los tiquetes aéreos, hospedaje, alimentación, desplazamiento, post operatorio ronda aproximadamente $85.000, se que no será fácil pero con la ayuda de cada uno de ustedes con lo que deseen donar les agradecería mucho, cada colón o dólar suma a la causa, tengo muchas ganas de vivir.

En Costa Rica hay una Asociación de Hipertensión Pulmonar en el cuál me han ayudado mucho desde la parte psicológica hasta en ayuda económica, a veces yo doy charlas de motivación para que mi testimonio les sirva a otros pacientes que padecen de esta enfermedad rara.
