My Life Between the Fight Against Cancer, the Strokes of Art, and the Hope of Becoming a Dad.*_

Hello, I am Marvin Yorlian Mirabal M, I am 22 years old. I have faced great challenges, including leaving my country in search of opportunities. Art has always been my passion. With every line of my drawings, I express the gift God gave me, turning paper into a reflection of my soul.

But today, I write to you not just as an artist, but as a young man facing his third and most difficult battle against cancer, while dreaming of becoming the best dad to my soon-to-be-born child.

My story with cancer began in January 2022, when I felt discomfort in my right testicle. After several tests, I was diagnosed with Classic Seminoma and underwent surgery (Removal of the testicle). Biopsy Report: Tumor formation confined to the testicular base, measuring 0.9 cm x 0.8 cm, with the presence of tumor emboli, tumor cells in the tunica albuginea, and altered tumor markers (AFP and HCG).

After the surgery and post-operative follow-up, it was determined that there were no secondary lesions. I thought the worst was over, but life had other plans. During my recovery, I dedicated myself to perfecting my artistic drawing techniques, and in the two years following my surgery, I developed work skills and personal challenges (as a tattoo artist and barber). Amid this process, I saw that opportunities for young people were limited, so I made the difficult decision to leave my country as a migrant. After a long and arduous journey through six countries, learning about their different cultures and working as a barber to support myself and continue my journey, I arrived in Mexico. There, I registered on the CBP (Customs and Border Protection) ONE app from the U.S. and waited for 10 months.

In August 2024, while waiting in Mexico, I noticed a small lesion on my forehead that grew rapidly. I received medical assistance at a health center, where I was referred to general surgery. At this point, my fears and suspicions returned, thinking about the cancer I had already overcome and I did not believe it could come back. After evaluation and medical tests, I underwent surgery on January 2nd, 2025, (Tumor removal) on my face (forehead), measuring 11 cm x 7 cm x 5 cm.

Biopsy Report: January 22nd, 2025, Malignant neoplasm histologically consistent with Metastasis of a mixed germ cell tumor located in the reticular dermis associated with lymphovascular invasion. I received a devastating diagnosis: the cancer had returned more aggressively. Additionally, due to persistent coughing and severe pain, I was evaluated by pulmonology, where a chest X-ray revealed a giant tumor lesion in the right hemithorax.

A contrast CT scan of the chest reported:

1) Mediastinal tumor causing displacement of mediastinal structures and reduced bilateral lung volume.

2) Collateral circulation due to superior vena cava obstruction.

3) Paraneoplastic pleural effusion.

4) Axillary and cervical lymph nodes with infiltrative appearance.

Currently, these are the diagnoses I have based on X-rays and CT scans. A new diagnosis affecting my respiratory system has left me with coughing, difficulty breathing, chest pain, and back pain. We have begun a new battle where the oncologist has advised me to start treatment with Polychemotherapy with Etoposide on January 31st, 2025 for clinical improvement. On February 5th, 2025, I was discharged.

Discharge Diagnosis: 1) Recurrent metastatic advanced right testicular germ cell cancer with poor prognosis + Superior vena cava syndrome in remission + Oncological pain syndrome + placement of a PICC line + 1st cycle of chemotherapy.

I was discharged to continue recovery at home and referred to a nutritionist due to weight loss (5 kg) and palliative care.

However, in the following days, I began experiencing fatigue, exhaustion, 6 to 10 liquid bowel movements in 24 hours, nausea, vomiting, and fever. On February 19th, I went to the emergency room, where clinical tests reported: Hemoglobin: 10.7, Leukocytes: 0.52, Lymphocytes: 56, Platelets: 141,000, Electrolytes: Sodium 129, Chloride 89.

Chest X-ray showed:

Diagnosis: Pneumothorax.

Currently, I am undergoing chemotherapy, but the side effects have been severe: fatigue, nausea, and vomiting.

According to the CT scan, the diagnosis is Right-sided Hydropneumothorax.

Therefore, I urgently need the placement of a PleurX pleural catheter to relieve symptoms associated with pleural effusion and right-sided hydropneumothorax.

Despite everything, my spirit remains strong because I have a powerful reason to fight: my wife is 36 weeks pregnant, and I will soon be a dad.

**Mi vida entre la lucha contra el cáncer, los trazos del arte y la esperanza de ser padre.**_

Hola, soy Marvin Yorlian Mirabal M, tengo 22 años. He enfrentado grandes desafíos, incluyendo dejar mi país en busca de oportunidades. El arte siempre ha sido mi pasión. Con cada línea de mis dibujos, expreso el don que Dios me dio, convirtiendo el papel en un reflejo de mi alma.

Pero hoy, les escribo no solo como artista, sino como un joven que enfrenta su tercera y más difícil batalla contra el cáncer, mientras sueña con ser el mejor padre para mi hijo que está por nacer.

Mi historia con el cáncer comenzó en enero de 2022, cuando sentí una molestia en mi testículo derecho. Después de varios exámenes, me diagnosticaron Seminoma Clásico y me sometí a una cirugía (extracción del testículo). Informe de biopsia: Formación tumoral confinada a la base testicular, midiendo 0.9 cm x 0.8 cm, con presencia de émbolos tumorales, células tumorales en la túnica albugínea y marcadores tumorales alterados (AFP y HCG).

Después de la cirugía y el seguimiento postoperatorio, se determinó que no había lesiones secundarias. Pensé que lo peor había pasado, pero la vida tenía otros planes. Durante mi recuperación, me dediqué a perfeccionar mis técnicas de dibujo artístico, y en los dos años posteriores a mi cirugía, desarrollé habilidades laborales y desafíos personales (como tatuador y barbero). En medio de este proceso, vi que las oportunidades para los jóvenes eran limitadas, así que tomé la difícil decisión de dejar mi país como migrante. Después de un largo y arduo viaje a través de seis países, aprendiendo sobre sus diferentes culturas y trabajando como barbero para mantenerme y continuar mi viaje, llegué a México. Allí, me registré en la aplicación CBP (Customs and Border Protection) ONE de EE.UU. y esperé 10 meses.

En agosto de 2024, mientras esperaba en México, noté una pequeña lesión en mi frente que creció rápidamente. Recibí asistencia médica en un centro de salud, donde me refirieron a cirugía general. En este punto, mis miedos y sospechas regresaron, pensando en el cáncer que ya había superado y no creía que pudiera regresar. Después de la evaluación y los exámenes médicos, me sometí a una cirugía el 2 de enero de 2025 (extracción del tumor) en mi rostro (frente), midiendo 11 cm x 7 cm x 5 cm.

Informe de biopsia: 22 de enero de 2025, Neoplasia maligna histológicamente consistente con Metástasis de un tumor de células germinales mixto ubicado en la dermis reticular asociado con invasión linfovascular. Recibí un diagnóstico devastador: el cáncer había regresado de manera más agresiva. Además, debido a una tos persistente y dolor severo, fui evaluado por neumología, donde una radiografía de tórax reveló una lesión tumoral gigante en el hemitórax derecho.

Una tomografía computarizada con contraste del tórax reportó:

1) Tumor mediastínico que causa desplazamiento de las estructuras mediastínicas y reducción del volumen pulmonar bilateral.

2) Circulación colateral debido a la obstrucción de la vena cava superior.

3) Derrame pleural paraneoplásico.

4) Ganglios linfáticos axilares y cervicales con apariencia infiltrativa.

Actualmente, estos son los diagnósticos que tengo basados en radiografías y tomografías. Un nuevo diagnóstico que afecta mi sistema respiratorio me ha dejado con tos, dificultad para respirar, dolor en el pecho y dolor de espalda. Hemos comenzado una nueva batalla donde el oncólogo me ha aconsejado comenzar tratamiento con Poliquimioterapia con Etopósido el 31 de enero de 2025 para mejoría clínica. El 5 de febrero de 2025, fui dado de alta.

Diagnóstico de alta: 1) Cáncer de células germinales testiculares derecho metastásico recurrente avanzado con pronóstico reservado + Síndrome de vena cava superior en remisión + Síndrome de dolor oncológico + colocación de una línea PICC + 1er ciclo de quimioterapia.

Fui dado de alta para continuar la recuperación en casa y referido a un nutricionista debido a la pérdida de peso (5 kg) y cuidados paliativos.

Sin embargo, en los días siguientes, comencé a experimentar fatiga, agotamiento, de 6 a 10 evacuaciones líquidas en 24 horas, náuseas, vómitos y fiebre. El 19 de febrero, acudí a urgencias, donde los exámenes clínicos reportaron: Hemoglobina: 10.7, Leucocitos: 0.52, Linfocitos: 56, Plaquetas: 141,000, Electrolitos: Sodio 129, Cloruro 89.

Radiografía de tórax mostró:

Diagnóstico: Neumotórax.

Actualmente, estoy recibiendo quimioterapia, pero los efectos secundarios han sido severos: fatiga, náuseas y vómitos.

Según la tomografía, el diagnóstico es Hidroneumotórax derecho.

Por lo tanto, necesito urgentemente la colocación de un catéter pleural PleurX para aliviar los síntomas asociados con el derrame pleural y el hidroneumotórax derecho.

A pesar de todo, mi espíritu sigue fuerte porque tengo una poderosa razón para luchar: mi esposa está embarazada de 36 semanas, y pronto seré padre.

¿Por qué necesito su ayuda?

Los costos de mis medicamentos, nutrición, transporte y cuidados paliativos son abrumadores. Además, mi esposa es mi único apoyo cercano, y pronto tendremos a nuestro hijo, lo que aumenta nuestras necesidades.

- Continuar mi tratamiento de quimioterapia.

- Cubrir medicamentos, nutrición, transporte y exámenes médicos.

- Adquirir un catéter PleurX para aliviar los síntomas del derrame pleural debido al hidroneumotórax derecho.

- Superar este proceso junto a mi esposa y recibir a nuestro hijo.

Mi Sueño

Mi mayor deseo es vencer esta enfermedad para poder abrazar a mi hijo, enseñarle a dibujar y verlo crecer.

Quiero seguir creando arte, cortando cabello como barbero y devolverle al mundo toda la bondad que recibo hoy.
