Leody Diaz Jr.

We are organizing a fundraiser to collect donations in honor of Leody Diaz Jr (the brother of our nurse practitioner Jen Diaz), who unexpectedly passed away on 10/1/23. Leody, was supporting Jen's entire family in the Philippines, who all live in their family home together- which is a Filipino tradition. Their family has struggled with poverty since her father's death at a young age. Jen's sister, younger brother, nephew, and her mother all were financially dependent on Leody's income to afford their home and expenses. In lieu of flowers, we ask you to consider a financial donation to help Jen and her family pay for the costs of the funeral and to assist them during this time of need and great tragedy.

We request that all donations be anonymous and note that all funds raised will be directly to given Jen and her family.
