Rooted, A Wholeway House

Rooted is a re-entry initiative designed build a continuum of care that bridges confinement to community, and adolescence to adulthood.

Our mission is to provide adjudicated youth, navigating the critical transition between the juvenile and adult justice system, with a safe, therapeutic, and holistic reentry environment. Through comprehensive wraparound services that address emotional, educational, social, and vocational needs, we empower young people to heal, grow, and build a sustainable path toward a positive, self-directed future.

As therapists at Giddings State Juvenile Correctional Facility, Avery Nelson (LCSW) and Jamie Falco (LPC) worked with youth inside the system to grow and heal only to watch them step outside the gates and into adulthood with no guidance, no access to services, and no preparation. The gap was devastating, and the outcomes were predictable.

We founded Rooted to change that story.
