treatment for rare disease

I have a very rare disorder called adiposis dolorosa a form of Dercum disease. Most doctors have never heard of it and although I have had it for five years, I only recently received the correct diagnosis. I have had the required MRI, Ultrasound and xrays. I have expert doctor's reports indicating that surgery is medically required.
I am retired. have no family and have a limited, fixed income. There is no surgeon in Canada familiar with the treatment for this very rare disease. The only group I have been able to find, expert and least expensive is in Madrid, Spain. The cost of transportation, treatment, hospital etc. is approximately $12,000.00 CDN which I do not have.
The condition is extremely painful on standing and walking and interferes with every aspect of my life. I live alone and have no assistance available.. I presently have to use two canes for support and there is lttle available to treat the chronic pain. The surgery involves removal of abnormal fatty tissue around the knees and at the top of the thighs, 
Any help would be more than appreciated.
