Jeremias Velazquez January 24, 1977 - November 28, 2022 born in Chicago, Illinois to Ismael and Hilda Velazquez, brother to Jesus Olmeda, Lillian, Carlos and Keren Velazquez and uncle and cousin to many. Jery was 45 years young who was full of life but unfortunately his heart was too weak and he needed a heart transplant that his body just couldn’t wait for. Jery is survived by his son Jakob Velazquez who needs our help to cover his funeral services. Please help him honor his father and give him a proper goodbye with a donation.
