It is with a heavy heart that I come to you to ask for help on behalf of the family of Phillip Smith, AKA Jake Brake. His passing was quite unexpected, and his family was left unprepared. He left this earth on June 24th, 2015, and he leaves behind his wife Sara Smith and five children, Trucker Smith age 3, Zoie Orduna age 8, step-daughter Lexis Moore age 10, Zavery Nichols age 11, Meagan Smith age 14, and Brycelynnr Smith age 16.
