I'm in need of a ramp into my house. Any help is appreciated

I'm Jimmy and I have Parkinson's disease, GERD, and other health issues. I'm in need of a ramp going into my house because I can't pick my feet up more than 2 to 3 inches, and I can't walk normally anymore. They call it the Parkinson's shuffle; it's how I walk now, using a cane. On good days, I can somewhat make it up without help, and that's only 5% of the time.

It will also benefit my mom, Judy Sturgill, as she's getting older and has problems with the steps too. I thought my insurance would help with it, but it's called housing modification, and it won't help us. Anything you can help me with will be greatly appreciated, and I will share photos of the ramp once it's finished. Thank you very much for your time and help.
