On September 9, 2025, at 6:21 PM, my mother, Clare Celano, passed away after a long and challenging medical journey. Her time in the hospital was filled with ups and down, painful moments of struggle, but also beautiful moments of resilience and accomplishments that many thought were impossible. Each day, Mom woke with strength and courage, determined to fight and endure months of challenges with incredible grace.
Clare was so much more than a patient—she was a devoted mother, an accomplished journalist, a cherished friend, a caring nurse of more than thirty years and a beautiful soul who touched every life around her. She loved deeply, laughed often, and gave endlessly. Her radiant smile will never be forgotten.
As we say goodbye, I am carrying forward her story and her spirit telling her story and demanding accountability by creating a documentary with the real life story of her medical journey and building The CCH Foundation (Clare Celano Humanitarian Foundation). The charitable foundation is dedicated to effecting change in healthcare focusing on mental health, patients with disabilities, and accountability within hospitals, subacute facilities, long-term acute care centers, and nursing homes.
No family should ever feel bullied for advocating for their loved one. Doctors must recognize that families often know their loved ones best. Patients deserve access to their own therapists and psychiatrists—especially because trust is not easily given, and for many, it is the foundation of healing.
Our Mission
The mission of the CCH Foundation is to advocate for and support patients by ensuring they have access to the resources, information, and care they need to navigate their healthcare journey.
We are committed to empowering individuals and their families, promoting patient rights, and bridging gaps in healthcare systems.
Through education, support services, and collaboration with healthcare professionals, we strive to create a more compassionate, equitable, and understandable healthcare environment for all patients.
Any contribution, no matter the size, will help us honor Clare’s memory, launch the CCH Foundation, and ensure her fight creates lasting change for others.
