Hello, I'm Josh. I was born with spastic quadriplegic cerebral palsy. I am 25 years old. Currently, I am attending Kalamazoo Valley Community College to study video game art and animation design. I am working to earn a degree in Art in New Media.
As a result of my CP, I use a power wheel chair for mobility in the community and in my apartment. I am unable to walk and unable to use my right hand. I depend on my left hand for driving my power wheelchair and all my daily activities. At home, I have caregiver assistance 16 hrs/day. Unfortunately, the equipment that I depend on for daily use is worn out and broken. I am urgently in need of a new shower/toilet chair for my bathroom and a new lift to help my caregivers transfer me from my wheelchair to my bed and the toilet. Both my old lift and my old shower/toilet chair are over ten years old, too small for me, and broken. I have contacted my insurance over the past five months trying to get new equipment, but they will not cover them. My physical therapist has helped me write nine revisions of letters of medical necessity. However, the insurance company has still only agreed to pay for $110.17 of the shower/toilet chair price of $2,600 and for $1283.40 of the lift with transfer sling price of $3,600. This leaves me with $4,806.43. In order to cover this cost, I have to pay out of pocket. I am unable to afford this based on my social security income. I am hoping you can help me begin to cover this cost for a home lift and shower/toilet chair.
