Help with transportation n other medical needs

Hi everyone, I'm fundraising for myself.  Ambulette service fees is alot n need help. I'm having surgery next month in stony brook, which I need to see the surgeon for pre op prep. See my primary doctor for medical clearance,maybe twice for office visit first cause I haven't seen my doctor in a long time, then medical clearance appt. Few days before surgery, I need to be swab for covid n then to the hospital for surgery. Those 5 ambulette services is what I need help with n other  medical co pays that I'm backed up with.
