
Help with physical therapy costs (and other medical expense)

About a month ago, i slipped in the kitchen, and when i fell, i sprained a muscle in my hip and in my shoulder (from trying to catch myself) so badly that i needed an ambulance. I've been out of work since, and i have no PTO to cover my time. My short term disability insurance only pays a small portion of my salary, and it's taking a long time to arrive, as it's coming in small installments.
I took out a loan from my 401k to cover my rent and utility bills for the time being, but i still need to be able to pay for physical therapy so that i can get back to work as soon as i can (and i also have bills for the ambulance, the emergency room, and the 3 visits ive made to my PCP this month). The UPMC emergency room didn't refer me to PT, they just took xrays and a ct scan, then gave me some crutches and sent me on my way
I have health insurance, but my deductible for in-network care is over $5000, so all these visits have to be paid out of pocket.
I'd appreciate any help that anyone can give me.
