For an organ transplant, they won't know until they cut me open if just my liver and kidney need a transplant, but a more complex surgery might happen. Still, they and I won't know which one until they look to see which one and won't know until I wake up from anesthesia after surgery and which surgery I had; when I'm coherent again after the anesthesia wears off, I guess I'll be in pain for a few days...
