Help Shaun Overcome Medical and Legal Hurdles

Hello,
It's Shaun. I'm incredibly embarrassed to have to even do one of these but life has a tendency to swing in some wild and chaotic ways.
In Nov I had managed to pinch my sciatic ( or so we had thought at the time.) I thought it was just a run of the mill injury akin to countless others
Fast forward a bit and to show how this ties together.
On Feb 22 I fell in the outdoor lot of my work. Initially I thought it was just an aggravation of the sciatic issues I had had for months before. Let it ride out for a week while realizing it isn't getting any better. Ash drove me to the er, with pain bad enough I had to ride in the back of her jeep. After getting a CT scan it became apparent that I had herniated one of my disc's and it was causing my sciatic nerve to be pushed into bone requiring a specialist referral and eventually surgery.
I had tried to do everything as correct as I could and thought I was rock solid for a short term disability claim kind of like what I utilized for previous surgery.
As it turns out even with the best laid plans life has a habit of kicking me in the teeth pretty well from time to time. My disability claim has been denied and after a conversation with workforce safety. It has become apparent that my employer is claiming that my fall didn't occur on their property and they failed to report either instance. Considering my previous bad luck and what was I was told about the situation, I'm not real hopeful that it is going to go in my favor.
So I'm here asking for some help to try and get caught up. I've been out of work for just about 3 months and am in post surgery recovery. Still unsure what the post surgical life looks like since I haven't had my follow up yet.
The money would be utilized paying some of my medical bills, getting caught up on the mortgage and anything left over would likely be used to fund legal services.
Thank you very much.
