Help Richard Overcome Health and Financial Struggles

My name is Richard, and I am 52 years old. Due to a number of serious health conditions, I am currently unable to work and am facing significant financial and physical challenges. I hope this message will help convey my situation and the support I am seeking.
In 1998, I developed severe cellulitis in my leg, which almost led to the loss of my leg from the knee down. Fortunately, doctors were able to save it, but I continue to experience recurring infections whenever I get even a minor scratch. In 2001, I was diagnosed with a brain tumor, which has contributed to ongoing weight gain. Then, in 2008, I experienced a life-threatening incident at work where I essentially stopped breathing. I was diagnosed with severe sleep apnea and placed on oxygen. Despite the doctor's advice to stop working, I continued as best I could until 2012, when my physical condition deteriorated to the point where I could no longer walk significant distances.
Today, my mobility is extremely limited. I can only walk short distances, and for anything over 20 feet, I require a walker or wheelchair. When I need to go to a store, I rely on a motorized cart. Over the past five years, my brother has been my primary caregiver, helping me with basic daily tasks such as getting in and out of bed, showering, and even using the toilet. He is unable to work due to his caregiving responsibilities.
I have applied for disability several times, but I have been denied four times, with the latest denial citing insufficient documentation. Unfortunately, I cannot drive; I gave up my driver's license in 2016, and we do not have access to a car. My sister generously allows us to use hers for grocery shopping, but even that is difficult. A typical grocery trip involves a 70-mile round trip, and after each trip, I am bedridden for several days due to the physical pain.
I can't imagine how difficult it would be to regularly make a 160-mile round trip to see doctors for my ongoing medical needs. The physical toll this would take on me is overwhelming, especially given the current strain of even a 70-mile trip.
I have exhausted my savings and am now facing the very real possibility of not being able to cover basic living expenses, such as utilities (power and water). In about four months, my savings will be depleted, and I will be unable to pay these bills.
I am reaching out to ask for any possible assistance. Specifically, I would greatly benefit from help with covering my essential living expenses and securing a small deck and ramp to allow me to get in and out of my home more easily. My brother, who has been caring for me without compensation, is also under a great deal of stress, and we are both uncertain of how we will manage going forward.
If there is any way you can assist or provide guidance, I would be deeply grateful. Thank you for taking the time to read my situation, and I look forward to any support or advice you can offer.
