My Bday in the hospital!! I really need some help

My name is JJ and I live in Carlsbad with my girlfriend & 3 dogs and I am in some pretty desperate times. I was in a car accident six years ago and I am paralyzed from the shoulders down and I am in real need of your help. I am sad to say that I am really struggling to pay my way in life right now.. Finding caregivers is next to impossible with my budget!! I feel like I am hanging by a thread, my girlfriends steam is running out, I am mentally and physically suffering and this fundraiser would change our lives forever. The amount I am putting up is based roughly on 2 years expenses:

-Rent $2,880
-ADAPT $2400/mo (specific spinal cord injury rehabilitation)
-Food $300-$500/mo( The government says I make too much money from the $1500 disability I get to qualify for EBT card!!!)
-Internet/Cable $241/mo
-Phone bill $134/mo
-New bed $5k-$8k
-Adaptive equipment/Repairs (mouth sticks, mounts, computer, certain supplies, etc.)
-CC Debt $418/mo (accrued over last 5 1/2 yrs)
-Enabling me to save and get my finances straight to buy a home and find a steady income stream
