Help Mark with medical and handicap Modifications

Hello, My name is Caleb and I am looking for help for my father. Last April my father (Mark) collapsed in his home and while he was fully conscious he was stuck on the ground sitting on his legs. After a day of not being able to reach him i made the 3 hour drive to his house. He was rushed to Darthmouth Hospital in New Hampshire. He had multiple organ failure, septic shock and the doctors gave him a 20% chance of making it through the first night. He did make it through that first night and the next day they amputated above the knee on his right leg because of lack of blood flow from sitting on it when he collapsed. He woke up a few days later and thats when his real struggle began. He spent over 30 days in the icu until he was finally well enough to be moved to a rehab facility. He was moved to Fairlawn in Worcester, Ma to be closer to my sister and myself while in rehab. he spent 3 months there until he was moved again to another rehab in Sudbury,Ma because Medicare refused to keep paying for Fairlawn because it was a much more intense rehab even though he didn't even receive his new prostheses. We filed multiple appeals to medicare to try to keep him at Fairlawn so he could get the rehab he needed with his new leg to no avail. After about 6 weeks in Sudbury he finally received his new leg AND THE NEXT DAY HE WAS TOLD HE HAD TO LEAVE BY THE FOLLOWING MONDAY. Medicare said he was ready to go home because he had his new leg. We again filed appeals knowing that he needed as much time as possible in the rehab with his new leg to learn how to walk again. All appeals were denied and he was forced to leave with one day of pt on his new leg. Mark just turned 65 a month after the start of this and just got on medicare. Never had a claim before and they and his insurance company did whatever they could to get him out. Mark lives up in Vermont in a small house on the side of a mountain which is the farthest thing from handicap accessible there is. After being home for two weeks after his 5 month stay in the Hospital and rehabs he received a letter from medicare saying " sorry Mr. Harmon we were under the impression you could walk 100 steps unassisted and thats why we stopped paying for your care" even though we appealed it multiple times. Next surprise comes with at home PT. He had a visiting nurse 2 times a week for at home PT to help him learn to walk again. One week he changes one appointment because he is going to see a friend in Maine, another friend will be bringing him because he is still wheelchair bound and can't drive and they instantly kick him off of at home PT because he is able to leave his house is their reasoning. He has been through the ringer with insurance companies and that is why we are here today asking for help. His prosthesis was not fully covered by insurance for some reason they said the company was "out of network" but yet the rehab set it up. He currently owes just under $7,000 for his leg, $5,000 in other hospital and medical bills, and just can't afford to pay the bills and make the necessary changes to his home and vehicle so he can be safe and comfortable. He is on social security and has a extremely tight budget and has no other options then to ask for help.
